Frontier Hospice proudly serves patients in Pueblo and Fremont counties in Colorado.

Our Hospice Services

Hospice offers anyone the choice to complete their lives with dignity, purpose and peace. Our goal is to help alleviate pain and symptoms while providing education, emotional and spiritual support to the patient and loved ones. Individuals and their loved ones choose hospice care when the goal of treatment has changed from curative to comfort care.

Our local hospice care professionals have over 100 years of combined hospice experience.

Frontier Hospice utilizes an interdisciplinary team approach to end-of-life care. All services are supervised by physicians, registered nurses and licensed social workers.  

Our patients and their loved ones help us create an individualized plan of care. The plan of care focuses on the providing for physical, emotional and spiritual needs.

Who Pays for Hospice?

In most cases hospice is 100% paid by Medicare an/or Medicaid

Many private insurance companies now offer coverage of hospice benefits.

Patients without a payment source will be considered for services with “no charge”.

Frontier Hospice provides a NO COST evaluation to determine if hospice services are appropriate.

Hospice pays for

Durable Medical Equipment – oxygen, bed, walker, etc.

Medication – associated with the terminal diagnosis necessary for the patient’s comfort..
